Bluewater Medicine will close permanently after December 21, 2020. 

The decision to close my office was very difficult and was based on many factors.  Importantly, I no longer have the energy that I once had over two decades ago when I first opened this practice.  During retirement I plan to travel, spend more time with my family, and explore hobbies and endeavors outside of medicine.  

It is important for you to make arrangements to continue to receive quality medical care.  If you need assistance in finding another physician, I suggest that you contact your health plan and request a list of physicians in your plan’s network or call Twin Cities Hospital Referral Line at (850) 729-9468.

Once you have a new physician, he/she will need copies of your medical records in order to care for you. I cannot release copies of your records unless I have your written authorization to do so.  Please complete this form and send it to me as soon as you know who your new physician will be.  Note that records will not be available just by stopping by the office as we will require time to assemble the records and transfer them to a CD.
